Citizen's panel - register your interest

Please give your details if you would be interesting in joining a 'citizen's panel' to evaluate the criteria for relocating physiotherapy services.  

We will contact you with more information as soon as possible.  

Please note that the panel will be selected to offer a fair representation of the people affected by changes to services, by taking into account factors like where you live, your age and background.  This means that not everyone who registers an interest will be able to take part.
