Bradford District and Craven group of GPs

The Bradford District and Craven Health and Care Partnership are collecting survey responses on behalf of our Primary Care Networks (groups of GP practices - PCNs).

Your individual response stays confidential and no personally identifiable data will be collected or shared. Anonymised survey responses will be exported for the purpose of analysis and reporting.

PCNs and the Health and Care Partnership will receive a report on the aggregated survey findings and anonymised data from the survey.

A survey is available in British Sign Language on the right of the page.

    Proposed enhanced opening hours in GP Practices

    GP practices are working together to look at how services are provided outside of the core practice opening hours of 8:00am - 6:30pm. To inform this we would like to understand the views and needs of our patients in regard to accessing services at alternative times.

    We want to hear what is important and what matters to you when you need one of our services. Please answer the questions based on your own views and experiences, and encourage other people who access our services to take part.

    It is very important that you follow the question instructions to ensure you complete the survey correctly. For example, some survey questions are single choice while others are multiple choice.
